Over the past few decades, probiotics have dominated emerging research regarding the importance of microbes in regulating human health. Having evolved over millions of years alongside our immune system, we have come to depend on them for our health as much as they have come to depend on us. Meanwhile, prebiotics are well known and have been studied for their necessary role in feeding probiotic species. More recently, research has focused on a group of related compounds known as postbiotics.

By definition, postbiotics are not live bacteria, but rather bioactive compounds secreted by probiotics. Postbiotics represent a new category of therapeutic intervention. The significant impact of postbiotic metabolites on the microbiome helps explain why a relatively small amount of probiotics can have such a profound impact on the expansive microbiome, with effects on gastrointestinal, immune, and systemic health. Postbiotics share many health benefits with prebiotics and probiotics, but they do not require fermentation, survival, or overcoming other barriers to metabolism. Clinical trials of postbiotics have demonstrated a multitude of health-promoting activities, ranging from metabolic regulation and weight management to immunomodulation and antioxidants. For example, Lactobacillus plantarum L-137 is a heat-killed postbiotic that has extensive in vitro and in vivo data demonstrating its ability to support the immune system by increasing innate and acquired immunity. L-137 increases interferon beta levels, improves host defense against influenza, and significantly reduces the incidence of upper respiratory tract infections (URTIs) in stressed individuals. L-137 has also been shown to decrease markers of inflammation such as ALT and improve lipid metabolism, such as total cholesterol and LDL cholesterol levels, in healthy individuals with high BMI and baseline inflammation.

Similarly, Lactobacillus paracasei MCC-1849 is a heat-killed postbiotic that has been shown to reduce cold incidence, symptom scores, and total days with symptoms in susceptible individuals, and may improve antibody responses in older adults. Similar to L-137, MCC-1849 appears to modulate the immune response through increased interleukin 12 (IL-12) and downstream effects such as natural killer cell activation. It is clear that postbiotic preparations have the ability to activate and modify immune cell function. Unlike probiotics, postbiotics provide the bioactive compounds necessary to deliver these benefits even if microbiome function is disrupted by conditions such as irritable bowel syndrome, small intestinal overgrowth, or dysbiosis.

The most interesting and arguably most important characteristic of a postbiotic is its quorum-sensing ability. Quorum sensing refers to the communication tactics of bacteria within and between species using signaling proteins. It is responsible for preventing the colonization of pathogenic bacteria and helps regulate and balance microbial populations within the microbiome. Direct supplementation with postbiotics provides a greater amount of these quorum-sensing signaling proteins, thereby improving microflora communication to regulate immune responses and bacterial numbers.
